Waiver & Privacy
Please take a moment to read each of the following statements:
If I experience pain or discomfort during the session, I will immediately inform my therapist so that
pressure/strokes can be adjusted to my level of comfort. I will not hold my therapist responsible for any
pain or discomfort I experience during or after the session.
I understand that the services offered today are not a substitute for medical care. I understand that my
therapist is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical
or mental illness.
I affirm that I have notified my therapist of all known medical conditions and injuries. I agree to inform
the therapist of any changes in my health and medical condition. I understand that there shall be no
liability on the therapist’s part should I forget to do so.
I understand that bodywork is entirely therapeutic and NON-sexual in nature.
By creating login information I attest to this release, I hereby waive and release my therapist from any and all liability, past, present,
and future relating to massage therapy and bodywork.
I understand that should I cancel an appointment less than 24 hours before the scheduled time or “no
show” an appointment, I am subject to a fee equal to the cost of the missed appointment. If the
appointment was booked under a gift certificate, it will be voided in lieu of the fee.

TERMS AND CONDITIONS:
You understand that the massage given to you by Wanda's Health massage therapy LLC is for the purpose of (stress reduction, pain reduction, relief from muscle tension, increasing circulation, or specific reasons stated on client intake forms).
You understand that the massage therapist does not diagnose illness or disease and does not prescribe medical treatment or pharmaceuticals, nor are spinal manipulations part of massage therapy.You understand that massage therapy is not a substitute for medical care and that it is recommended that you work with your primary caregiver for any condition you may have.
You have stated all my known physical conditions and medications, and I will keep the massage therapist updated on any changes.
Information and Suggestions:
-Prior to your massage, please remove contact lenses and all jewelry. Pull long hair back with a clip or band.
-In general, massage is given while you are unclothed. However, you may choose to wear clothing (loose,
comfortable clothing will not interfere with your treatment). You will be covered with a top sheet throughout
your session. This is your massage and you should be as comfortable as possible.
-Feel free to ask your therapist any questions before, during, or after the session. Your therapist is a highly trained
healthcare professional and wants to make you feel informed and comfortable.
We, the therapist, agree to perform my duties to the best of my ability keeping client health, safety, well-being and
satisfaction foremost in mind. We will keep all client information and details in strict confidence as indicated by law and
good ethical practice.
Collection of Information
Any and all personally identifiable information you provide to WHMT LLC through this Site is provided on a voluntary basis, and how much you disclose is up to you. The personally identifiable information collected on these pages may include, but is not limited to, name, title, organization, mailing address, email address, phone and fax numbers, and credit/debit card information. We also may collect information on your views and preferences as they relate to our services.
In some cases, such as blogs or public forums, any personal information you provide may become publicly available to other members and possibly the general public. When you voluntarily disclose such personal information, that information may be collected and used by others, resulting in unsolicited messages from others.
Unless otherwise expressly stated, no confidential relationship is established when any user of this Site voluntarily provides comments, e-mail addresses, and/or other information to WHMT LLC or posts the same in conference rooms, on profile pages, or in other interactive programs and features that may be developed and placed on-line from time to time. Such information is considered non-confidential, and WHMT LLC is free to reproduce, publish, or otherwise use such information for any purposes whatsoever.
We also may record information about the computer you are using to connect to the Internet (IP address) and about your browser.
Use and Disclosure of Information
We store and use the information you provide about yourself when placing a request or order or to complete the applicable transaction. We may share the information you provide with business partners or other outside parties to deliver content, materials, and services available on or through the Site to you or to the extent necessary to complete an applicable transaction or for any other lawful purpose.
We also may use information you provide to conduct other business or to contact you about WHMT LLC news, activities, membership, surveys, products, and services, and for other legitimate and lawful business purposes. If you decide at any time that you no longer wish to receive communications from us, please follow the unsubscribe instructions provided in any of the communications. However, WHMT LLC expressly reserves the right to contact any member or other visitor regarding account status, renewals, or other matters relevant to subscription or service.
We use non-identifying and aggregate information collected to generate statistical information, monitor and analyze demographic information and site traffic and usage, and improve our website. WHMT LLC reserves the right to study user's’ movement throughout, and use of, this Site and to inform advertisers of this movement and usage.
